Player Registration Form	This form must be turned in from all players prior to January 23rd . For more information about player registration click here.
All-Star Registration Form	This form is required at the end of the regular season from all players that are interested in being considered for All-Star selection.

Coaches Application Form

This application form is mandatory for all head coaches, assistant coaches, and team parents.

The Vestavia Hills Park and Recreation is requiring a background check on all prospective head coaches. Assistant Coaches and team parents are also subject to a background check. These applications will by screened by the park director or his designee. Thank you for your understanding as we continue to improve our league.

Player Evaluation Form

All coaches must fill out this player evaluation form for their players. This from must be filled out and brought to the All-Star selection meeting.
